Faglige interesser

Mine primære interesseområder ligger innen affektteori, med særlig fokus på affektorganisering og affektintegrasjon, samt prosess og utfall i psykoterapi. Disse to interesseområdene kombineres i både mitt kliniske arbeid, psykoterapiopplæring og veiledning, og i forskning. Fokus for forskningen er særlig på relasjonen mellom affektintegrasjon, affektorganisering og psykopatologi, samt betydningen av affekt og affektintegrasjon i psykoterapeutiske prosesser. Mer generelt er forståelse og systematisering av en rekke aspekter ved psykoterapiprosesser og betydningen av disse for utfall av psykologisk behandling viktige fokusområder i mitt arbeid, både klinisk og i forskning. I tillegg er jeg i mitt arbeid opptatt av effekt og effektivitet av psykoterapi i ulike pasientgrupper, særlig har jeg de siste årene arbeidet med og forsket på prosess og utfall i terapi med pasienter med såkalte "behandlingsresistente lidelser", altså pasienter som har forsøkt flere typer behandling uten å ha tilfredsstillende effekt av det.

I et mer rent affektteoretisk perspektiv er jeg opptatt av følelsenes fenomenologi, grunnleggende dimensjoner i det affektive rom, samt intersystemiske relasjoner mellom affektive-, kognitive-, motoriske-, sensoriske-, perseptuelle- og driftsbaserte systemer.

Det kan også nevnes at jeg også interesserer meg for og har jobbet en del med hypnotiske fenomener, både klinisk og i et mer eksperimentelt perspektiv.

5.) Utvikling av "The Affect Integration Inventory" (AII), et selvutfyllingsverktøy for måling av affektintegrasjon.

6.) Utvikling av "The Bodily Affective Mobilizability Scales" (BAMS), et observasjonsverktøy for identifikasjon av tverrstripede muskelresponser ved affektiv aktivering.

Bakgrunn

Utdannet Cand. Psychol ved Psykologisk Institutt, UiO. Klinisk erfaring fra poliklinikk, rusteam, samt døgnbehandling. Gjennomførte PhD som del av dobbeltkompetansestipend med parallelt fokus på forskning og klinisk arbeid. Psykologspesialist i klinisk voksenpsykologi, NPF. Gjennomført 3-årig videreutdanning i Affektbevissthetsmodellen (ABT), godkjent veileder og underviser i ABT. Gjennomført 3-årig core-training i Intensiv Dynamisk Korttidsterapi (ISTDP), gjennomført 3-årig utdanning som ISTDP veileder og -trener med Jon Fredrickson (Training for Trainers - ISTDP Institute), godkjent terapeut, veileder og lærer av ISTDP International.

Background: Equitable mental healthcare regardless of area of residence is a political aim in Norway. The purpose of this study is to evaluate geographical variation in private practitioner coverage and explore differences between private practitioners with public reimbursement and local community mental health centers (CMHCs) in terms of diagnoses and scope of outpatient treatment, with a focus on mood and neurotic disorders. Method: Cross-sectional registry-based study of adults in outpatient mental healthcare at private practitioners or CMHCs in Norway in 2014–2018. We calculated distribution of diagnoses, geographical variation in rates of private practitioners, and analyzed scope of treatment for patients with mood and neurotic disorders (ICD-10 disorders F3, F4). We estimate cost per treated patient and report geographic variation for hospital catchment areas. Results: Patients with F3, F4 disorders accounted for 64.7 % at private practitioners, and 41.5 % at CMHCs, and received more treatment at private practitioners, five more consultations in a 42 day longer period. There is considerable geographical variation in private practitioner coverage, highest in South-Eastern Norway and lowest in Northern Norway. Implications: As long as management and priority setting are differently organized for CMHCs and private practitioners in mental health care, we must expect geographical variation in access and use. It appears as an equity paradox that a group of patients, rejected by or not referred to CMHC, receive more treatment by private practitioners.

In a Norwegian study of 73 couples attending a residential couple therapy program lasting between 6 and 12 weeks, weekly self-report data on therapy alliance and couple satisfaction were collected using routine outcome monitoring (ROM). The aim was to show how dyadic analyses could be applied to examine the predictive association between alliances and couple satisfaction. Results showed that improved alliance between dyad members and their couple therapist predicted their spouses' alliance. The clinical implication of these findings should heighten awareness to the importance of male partners in couple therapy, something that predicts their spouses' couple satisfaction. These findings help nuance the already existing literature on the working alliance. Futhermore, we propose that dyadic analyses should be widely used in any psycho-therapeutic research that aims to understand the reciprocal effects of dyads.

In the present study we aim to increase our knowledge of the relationship between childhood trauma and outcome in couple therapy. We samples participants based on their sub-optimal responses to treatment as well as one member of the dyad having reported experiences of childhood trauma. Six participants constituting three couples were included. All data was analyzed using thematic analysis. Our main finding was that when couples are not able to agree ont he goal of their therapy and this is not handled adequately by the therapist, the alliance tends to split. The therapy thus becomes more individually focused at the expense of the couple relationship. Generally, participants did not themselves assess their past traumas as having negatively impacted therapy giving instead credence as to how it has impacted their self-knowledge. In the future, longitudinal studies should be conducted to explore if there is an association across treatment between trauma and the therapeutic relationship, and the influence it might have on outcome. The findings of this study further support the already existing literature on the importance of alliance and elaborates upon how split alliances occur, develops, and constrains therapy.

Purpose: Medication has been a central part of treatment for severe mental disorders in Western medicine since the 1950s. In 2015, Norwegian Health Authorities decided that Norwegian health regions must have treatment units devoted to medication-free mental health treatment to enhance service users’ freedom of choice. The need for these units has been controversial. The aim of this study was to examine why service users choose medica- tion-free services. This article examines what purpose these units serve in terms of the users’ reasons for choosing this service, what is important for them to receive during the treatment, and what factors lay behind their concerns in terms of medication-related views and experiences. Methods: Questionnaires were answered by 46 participants and 5 participants were inter- viewed in a mixed-method design integrated with a concurrent triangulation strategy apply- ing thematic analysis and descriptive statistics. Results: Negative effects of medications and unavailable alternatives to medication in ordinary health care were important reasons for wanting medication-free treatment. Medication use may conflict with personal values, attitudes, and beliefs. Conclusion: This study broadens the understanding of why the demand for separate medication-free units has arisen. The findings may contribute to making medication-free treatment an option in mental health care in general. To this end, clinicians are advised to communicate all treatment alternatives to service users and to be mindful of the effect of power imbalances in their interactions with them

Objective: To better understand the complexity of dyadic processes, such as the mechanisms of the working alliance, researchers recommend taking advantage of innovations in data analytic procedures when studying the interactions between therapists and patients that are associated with favorable therapeutic outcomes. Inspired by a recent line of alliance research using dyadic multilevel modeling, the present study investigated the hypothesis that convergence in the patient-therapist working alliance (i.e., increased similarity in ratings of the alliance across treatment) would be associated with better outcomes. Method: Data were retrieved from two samples: 1. A randomized controlled trial for treatment resistant depression (N= 96 dyads), and 2. An archival dataset of naturalistic psychotherapies from public health care (N = 139 dyads). Multilevel growth curve analysis was employed to investigate the degree of change in session-to-session agreement of global WAI ratings between therapists and patients (i.e., alliance convergence) as a predictor of symptom reduction in the BDI-II and the SCL-90R. Results: Contrary to our expectations, alliance convergence did not predict outcome in either sample, but was negatively associated with symptom severity in Study 2. Implications for understanding the complexity of dyadic processes and alliance work in psychotherapy are discussed.

Conflict with parents is common among depressed adolescents, interferes with treatment, and may increase risk of recurrence. Parental depressive symptoms have been shown to predict conflict with adolescent children, but an important role for different kinds of parental interpersonal problems, as described by interpersonal circumplex, is also plausible. This study compared parental interpersonal problems to parental depressive symptoms as predictors of parent-adolescent conflict reported by a depressed adolescent child, using multilevel linear regression, leave-one-out cross-validation and model stacking (N = 100 parents, 57 mothers and 43 fathers, of 60 different adolescents). Cross-validation and model stacking showed that including parental interpersonal problems contributes to accurate predictions. Parents reporting more interpersonal problems related to excessive dominance or submissiveness was associated with increased or decreased conflict, respectively. Parental depressive symptoms were found to be negatively associated with parent-adolescent conflict only in father-adolescent relationships.

Affect integration was operationalized through the Affect Consciousness (AC) construct as degrees of awareness, tolerance, nonverbal expression and conceptual expression of 11 affects. These aspects are assessed through a semi-structured Affect Consciousness Interview (ACI) and separate rating scales (Affect Consciousness Scales (ACSs)) developed for use in research and clinical work with adults with psychopathological disorders. Age-adjusted changes were made in the interview and rating system. This study explored the applicability of the adjusted ACI to a sample of 11-year-old children with internalizing problems through examining inter-rater reliability of the adjusted ACI, along with relationships between the AC aspects and aspects of mental health as symptoms of depression, symptoms of anxiety, social competence, besides general intelligence. Satisfactory inter-rater reliability was found, as well as consistent relationships between the AC aspects and the various aspects of mental health, a finding which coincides with previous research. The finding indicates that the attainment of the capacity to deal adaptively with affect is probably an important contributor to the development of adequate social competence and maybe in the prevention of psychopathology in children. The results indicate that the adjusted ACI and rating scales are useful tools in treatment planning with children at least from the age of 11 years.

Associations between symptoms and interpersonal problems, assessed with the Symptom Checklist–90 (SCL–90–R) and the Inventory of Interpersonal Problems–64-item version (IIP–64), are examined in a large psychiatric outpatient sample. On the basis of the IIP–64 scores, the sample was divided into 8 subgroups, made up of different types of predominant interpersonal problems. These octant groups were used as independent variables in analyses testing hypothesized associations with symptom subscales of the SCL–90–R. In general, strong associations between symptoms and interpersonal problems were found. In addition, hostile and paranoid ideation symptoms displayed significant differences among octant groups, and were associated with interpersonal problems of the vindictive/self-centered kind. Phobic anxiety was associated with interpersonal problems of the socially inhibited kind. Assessing specific combinations of symptoms and interpersonal problems might be useful in treatment planning and evaluation.

Monitoring of ongoing psychotherapy is of crucial importance in improving the quality of mental health care by detecting therapies being off track, which requires that the instrument used is psychometrically sound. This study investigates the psychometric properties of the Norwegian version of the Outcome Questionnaire 45.2 (OQ-45) and situates the results in an international context. Data from one non-clinical sample (n=238) and one clinical sample (N=443) were compared to international samples investigating reliability, cut-offs and factor structure. The results show adequate reliability and concurrent validity. The means, clinical cut offs, and the reliable change index vary across countries. However the means of the OQ for nonclinical samples correlates highly with external values of national well-being, indicating that the OQ is a valid instrument internationally. The factor analyses in the present study do not confirm the hypothesized factor structure of the OQ, but are similar to the results internationally.

Affective Change – Intensive Short-Term Dynamic Psychotherapy (ISTDP) Intensive short-term dynamic psychotherapy (ISTDP) is characterized by the therapist’s sustained effort to help patients get optimal access to feelings through the pressure for feelings, deactivation of defenses and regulation of anxiety. ISTDP is (a) intensive, in the sense that the therapist aims to help the patients to feel their feelings as deeply as possible, (b) psychodynamic, in the sense that the therapist is actively working with transference feelings and the function of the unconscious, and (c) short-term, in the sense that the therapist works to keep a maximum level of pressure, adapted to the patient’s capacity, with the aim of producing the shortest possible treatment with the greatest possible effect. The authors present the theoretical basis for ISTDP, a brief overview of research on the method, and an introduction to how the therapist works with helping patients access their feelings in ISTDP

The importance of affect regulation, modulation or integration for higher-order reflection and adequate functioning is increasingly emphasized across different therapeutic approaches and theories of change. These processes are probably central to any psychotherapeutic endeavor, whether explicitly conceptualized or not, and in recent years a number of therapeutic approaches have been developed that explicitly target them as a primary area of change. However, there still is important lack of clarity in the field regarding the understanding and operationalization of affect integration, particularly when it comes to specifying underlying mechanisms, the significance of different affect states, and the establishment of operational criteria for measurement. The conceptual relationship between affect integration and reflective function thus remains ambiguous. The present article addresses these topics, indicating ways in which a more complex and exhaustive understanding of integration of affect, cognition and behavior can be attained.

I mange år har det vært fokusert mye på ulike kvalitetsindikatorer innen psykisk helsevern. Det handler som regel om at frister må holdes, aktivitet må telles, prosedyrer må kodes, dokumenter må være på plass, og forskrifter og lover må følges. Med innføring av pakkeforløpene ønsker også myndighetene å sette en kvalitativ standard for tjenesten, blant annet for å sikre et mest mulig likeverdig behandlingstilbud i hele landet. Felles for flere av disse kvalitetsindikatorene er imidlertid at de er initiert ovenfra, med begrenset forankring hos de som gir eller mottar behandlingen. De omhandler også som regel administrative og organisatoriske forhold rundt tjenesten, og ikke kvaliteten på behandlingen per se. Som representanter for tjenesten og behandlere savner vi kvalitetsindikatorer som dreier seg om kvaliteten på innholdet i behandlingen: Opplever pasienten tillit til sin behandler? Er pasienten enig i målet med behandlingen og måten å jobbe med problemene på? Er symptomene på psykiske plager og problemer endret? Opplever pasienten bedre relasjoner, meningsfulle aktiviteter og mer mestring? Vi mener at kvalitet må undersøkes på andre måter enn vi gjør i dag, for å få svar på om vi gir god behandling til pasientene våre. Det er på tide å spørre pasienten på en systematisk måte hvordan de har det, og hvordan de opplever behandlingen. Det er også på tide å undersøke på gruppenivå om behandlingen faktisk virker. Eller mer presist: Hva virker for hvem? Hvem klarer vi ikke å hjelpe? Hva påvirker våre behandlingsresultater? Har endringer eller mangel på endring sammenheng med hvordan vi tilbyr og organiserer tjenestene?

Addiction programs are plagued with high dropout and relapse rates. A large proportion of patients suffering from addiction also suffer from personality disorders. A 30-day inpatient program based on intensive short-term dynamic psychotherapy was developed to address features of personality disorders such as anxiety regulation, emotion recognition, and handling of fear responses and projective processes. The hypothesis was that addressing comorbid symptoms of personality disorder might improve recovery from drug addiction. We used a pilot randomized controlled trial design with six-month follow-up of both cases and controls. Rates of remission, relapse and drop out were recorded at each time point. N-1 chi-squared (χ2) tests were conducted to examine the statistical significance of differences in outcomes in patients receiving the experimental treatment and controls. A control group of 20 patients and an experimental group of 42 patients were treated. Dropout: control group 40%; experimental group 23.8%. Sobriety at six months: control group 17.6%, experimental group 48.8%. Future study is warranted to examine intensive short-term dynamic psychotherapy’s long-term effects, study moderators of effects, and study its efficacy using a randomized controlled design.
